PA Act 168 Forms:
In accordance with the law that Pennsylvania Governor Tom Corbett signed on December 31, 2014, all prospective employees of SLSD that had direct contact with children at any current or former job must fill out an Act 168 form for each applicable job. If you are in need of a copy of this form, please see the attachment below.
